Online Artisan Survey

Artisan policyholders can complete their required survey through our Online Artisan Survey. This program is available for all new artisan policyholders as of October 1, 2009 and existing policyholders that renew on or after January 28, 2010. All other artisan policyholders should continue to call our Artisan Survey Unit at 1-800-456-4556 Option 1 to complete their survey.
